Education and its Assessment
Dr. Cadiz joined the KGI School of Pharmacy and Health Sciences in August 2015 from Palomar Medical Center in San Diego, CA, where she has been practicing as an inpatient transitions of care pharmacist. In this role, she works closely with members of the interdisciplinary health care team to optimize medication therapy and to provide education prior to discharge from the hospital. The main goals of the program are to empower patients and their families and to reduce hospital readmissions. Dr. Cadiz will continue her work with the inpatient transitions of care team at Palomar Medical Center. Additionally, she will be establishing outpatient transitions of care clinics in the ambulatory care setting and explore opportunities to improve care transitions from skilled nursing facilities.
Dr. Cadiz is a Board Certified Pharmacotherapy Specialist. She has a teaching background and holds her Master of Arts in Teaching from the University of California, Irvine. She earned her PharmD degree from the University of California, San Diego, and completed a PGY1 pharmacy practice residency at the University of California, San Francisco.
