The Patient Communication in Senior Adult Chronic Care IPPE is the second IPPE in the second professional year. This IPPE aims to build student awareness of the geriatric population, foster student communication skills, and provide residents with a medication action plan. Each student will be paired with two retirement community residents with whom they will visit on a bi-monthly basis. For sixteen weeks, a student will work with a resident to aim for the TARGET (To Achieve Reachable Goals and Effective Treatment) and promote resident’s self confidence in managing their medication regimen. Prior to each visit, students will attend bi-weekly class sessions to prepare for each week’s assigned activity through simulations and role-playing. Sample weekly assigned activities include: 1) conducting resident interviews; 2) reviewing medication regimens; 3) assessing medication adherence, and 4) providing disease prevention education.
